Spay/Neuter Certificate or Reimbursement Request
K9Lifesavers will cover the cost of spay/neuter at our partner vet(s) or reimburse you for the amount our partner vet charges us should you choose to spay/neuter at your own vet.
**Please note the rescue discount, in most cases, is significantly lower than your personal vet and Dogs in Danger Rescue Inc will only cover $80 per neuter or spay from your vet bill.
Request for spay/neuter:
Please use the below form to request a spay/neuter certificate for a puppy/dog adopted from our organization after June 14th, 2021. You can look up all the information needed to fill out the form, including the “spay/neuter by” date, on your copy of the adoption agreement received at the time of adoption. Please note that you will need the pup’s Dogs in Danger Rescue Inc given name at the time of adoption and rescue ID code located at the bottom of the contract to submit the form. Once submitted, a representative will send you a spay/neuter certificate for the partner vet you select so you can schedule the appointment directly. Once you receive the certificate, please contact the selected vet 5-8 weeks prior to the “spay/neuter by” date on your certificate. When scheduling your appointment please use the pup’s K9 Lifesavers given name and make sure you indicate you will be using a K9 Lifesavers certificate to pay for the surgery.
**Please note, Dogs in Danger Rescue Inc only covers the cost of spay/neuter and associated medicine. Anything additional, such as microchip or cone, is the financial responsibility of the adopter. K9 Lifesavers does not cover the cost of spay/neuter or reimbursement for spay/neuter for pets adopted from other organizations or puppies adopted from our organization prior to June 14th, 2021.
Request for reimbursement:
If you are opting to use your own vet for spay/neuter, please fill out the form below to request reimbursement of $80.00 by selecting the radio button “using my own veterinarian” under the Location section. At the same time, please submit a proof of spay/neuter receipt from your vet and request for reimbursement to spayneuterscheduling@Dogs in Danger Rescuelifesavers.org. In the email, please note the dogs K9 Lifesavers name so it can be connected to the form easily and ensure a timely reimbursement. If you do not hear back within 1 week period, please reach out to spayneuterscheduling@Dogs in Danger Rescuelifesavers.org to make sure your submission was received.
